Overview
Social Worker - LDA and Sensory Adult Social Care Team, Keynsham. You’ll join a team providing support to adults with learning disabilities, autism and/or sensory impairment, helping them navigate complex needs and access the right support. You’ll work with a broad network of professionals and have the opportunity to make a positive impact on the lives of service users and their families.
Your Role
As a Social Worker, you’ll manage a diverse caseload with a high degree of autonomy, supported by an experienced Team Manager. Your expertise and initiative will be valued, and your impact will be visible.
Responsibilities
* Leading adult care assessments and reviews under the Care Act 2014
* Designing personalised care and support plans
* Conducting Mental Capacity Assessments and Best Interests Decisions
* Managing risk through person-centred strategies
* Completing Section 117 Aftercare reviews and Continuing Health Care Checklists
* Leading on safeguarding enquiries
About You
We’re looking for someone who is passionate about empowering adults with diverse needs and committed to high-quality social work practice.
You’ll Need
* A recognised Social Work qualification (Degree, Diploma, CQSW or equivalent)
* Registration with Social Work England
* Experience supporting individuals with autism, mental health conditions, learning disabilities, or sensory needs and physical disabilities
* A Full UK Driving Licence and access to a vehicle
