We are recruiting a highly motivated, innovative and enthusiastic Band 5 Primary Care Staff Nurse to join our forward thinking, friendly and expanding Health & Justice team at HMP High Down. The role involves delivering healthcare in a secure environment with a 7-day week working model to ensure high standards of patient care.
Responsibilities / Main duties
* Provide an accessible individual patient assessment, treatment and referral service.
* Monitor the ongoing health status of patients and address identified issues.
* Influence health outcomes by working with patients to address lifestyle factors.
* Implement effective discharge plans for transfer or release locations.
* Coordinate multidisciplinary clinical practice to provide integrated care and treatment programmes.
* Collaborate with staff from other disciplines (e.g., officers and governors) to provide integrated treatment plans.
Key Tasks
* Triage patient needs in line with standard operating procedures and national policy (e.g., NICE).
* Assess, plan and implement care within each care/treatment pathway to promote wellbeing.
* Apply evidence-based practice to ensure effective and appropriate care.
* Carry out general nursing duties and maintain contemporaneous records.
* Support and contribute to the development of integrated healthcare.
* Act as a preceptor/mentor as appropriate.
Clinical duties include
* Assess, plan, implement and evaluate care for patients using appropriate directives or care pathways.
* Plan and organise own workload with minimal supervision.
* Administer medications as prescribed and manage related tasks (immunisations, injections, etc.).
* Refer to mental health or substance misuse services as needed and complete relevant paperwork.
* Manage self-harm risk assessments (ACCT) as required.
* Provide health promotion interventions and participate in health promotion/harm minimisation clinics (e.g., smoking cessation, immunisations, sexual health, chronic disease management).
* Take, collect or transport specimens as required; perform wound dressings; ensure discharge letters accompany patients returning to the community.
* Attend case conferences and provide cover for other teams when needed.
Education, qualifications and experience
* Essential: Registered Nurse Adult
* Desirable: Evidence of relevant CPD
* Essential: Previous experience in Primary Care services as a Band 5 nurse or student nurse
* Desirable: Prior experience in Primary Care within a prison setting
Skills and knowledge
* Effective communication with multi-disciplinary teams; ability to present cases and maintain clear handovers.
* Accurate and timely clinical records management (electronic and paper).
* Ability to discuss sensitive issues, provide supportive care, and challenge where necessary.
* Infection control, medicines management, and patient safety focus.
* Mentor, teach and support others; supervise team members; contribute to a positive team culture.
* Understanding of the Mental Health Act and safeguarding advocacy.
