Our client is seeking an Adult Social Worker to join their Young People Transitions to Adulthood Team based in Oldham.
What will your responsibilities be?
* Support young people preparing for adulthood, completing holistic assessments that identify aspirations, needs, risks, and the support required as they move from children's to adult services.
* Coordinate multi‑agency transition plans, working closely with education, health, adult social care, and community providers to ensure continuity of care and smooth handover.
* Promote independence and person‑centred outcomes, developing plans that build life skills, enhance wellbeing, and empower young people to make informed decisions about their future.
