Job overview

This post is for a registered clinician to work within the single point of access to undertake robust clinical triage and be a senior clinical decision maker, identifying the most appropriate clinical pathway for the patient to enable them to be seen in the right place, by the right professional and not be unnecessarily conveyed or admitted to hospital.

This post is maternity leave cover for 22.5 hours per week till 10th December 2026

Main duties of the job

The post holder will work in close partnership with the acute sector, social and community service colleagues. They will have in-depth clinical knowledge and experience, with high understanding of services and pathways to enable appropriate decision-making and problem solving with referrers to address patient’s clinical needs. This includes the management of clinical risk, coordination of referrals and be accountable for the appropriate utilisation of clinical capacity across the system.

The post holder will liaise at senior level with relevant medical, nursing and therapy staffing to ensure the patient is safely accepted onto the most appropriate clinical pathway, responded to by the most appropriate clinician and receives timely assessment, interventions and diagnostics as required. They may need to liaise with social services, voluntary sector, patients and their families as appropriate.

Person specification

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

10. Registered Allied health professional with HCPC or Registered Nurse with RCN or Registered ACP with appropriate registration body
11. Educated to diploma/ degree level or equivalent experience
12. Advanced history taking module or undertaking
13. Post graduate clinical training in an area relevant to the post

Desirable criteria

14. Recognised management or leadership qualification
15. Recognised teaching or mentorship qualification equivalent to level 3

SPECIAL CIRCUMSTANCES

Essential criteria

16. Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire

EXPERIENCE

Essential criteria

17. Experience of working as advanced clinical practitioner or advanced case manager
18. Significant experience of working in a community or inpatient setting
19. Experience in leading complex care co-ordination (including cognitive impairment) and developing highly specialised programmes of care with appropriate advice
20. Experience of interagency and partnership working with other professionals from the NHS, Social and Health Care and voluntary organisations

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria

21. Specialist knowledge in managing patients in the community with complex medical needs, long term/chronic disease management
22. Knowledge of governance and risk strategies and their application in a community setting
23. Knowledge of analytical and judgment skills, with the ability to interpret complex situation and make appropriate decisions
24. Well-developed interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and the public
25. Well-developed written and verbal communication skills
26. Strong IT skills including Microsoft Word, Excel and PowerPoint. Good presentation skills/public speaking.
27. Ability to work autonomously without supervision
28. The ability to work in emotive and highly charged atmospheres, meet deadlines and working under pressure
29. Ability to adapt to the demands of a constantly changing environment
30. Ability to challenge and be challenged
31. Proficient time management skills

Desirable criteria

32. Knowledge of current political drivers, health and social care issues
33. Budgetary awareness

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.
