Lead onboarding and clinical assessments for new clients entering Support at Home Develop personalised, goal-based care plans that promote independence Manage a caseload / funding with a focus on wellness, reablement and restorative care Coordinate services across nursing, allied health and support teams Provide clinical oversight to ensure safe, high-quality care at home Build trusted relationships with clients, families, GPs and providers Skills And Experiences About you You’re a confident, compassionate RN who thrives outside the traditional clinical setting. You enjoy autonomy, love building relationships, and take pride in delivering care that truly centres the individual. You’ll bring: Current AHPRA registration as a Registered Nurse Experience in aged care, community care, or case management Strong clinical assessment and care coordination skills A passion for wellness, reablement and independence Excellent communication and relationship-building ability What you’ll need Current Driver’s License First Aid & CPR Police Check (or willingness to obtain) Right to work in Australia Ready to take your nursing career into the community?
