An exciting opportunity has arisen for a Band 3 Community Support Worker to join the newly formed Treatment, Review and Assistance Team for Psychosis (TREAT). This post will support the development and delivery of a community-based clozapine initiation service, building on the success of the pilot programme in the Exeter area and supporting its expansion across all localities within the Trust.

Timely access to clozapine remains a clinically unmet need for many individuals with complex psychosis across Devon. This role will contribute to improving access to this highly effective treatment by supporting clozapine initiation in community settings, thereby reducing the need for inpatient admissions and enabling more people to benefit from early intervention.

The postholder will work as part of a multidisciplinary community team, supporting service users through the clozapine initiation pathway and contributing to the ongoing development of this innovative service model. This role offers the opportunity to be involved in a developing service that aims to improve outcomes, enhance patient experience, and support recovery-focused care for people with psychosis.

Main duties of the job

* To work under the direction of registered clinicians within the TREAT service to support individuals with complex psychosis receiving community-based clozapine initiation.
* To provide practical, emotional, and recovery-focused support to service users in line with agreed care plans.
* To assist with the delivery of physical health monitoring and observation processes associated with clozapine initiation, in accordance with Trust policies and training.
* To support engagement with service users and carers, promoting adherence to treatment and attendance at appointments.
* To contribute to the safe and effective running of the community clozapine pathway, including documentation and communication with the wider multidisciplinary team.
* To support the ongoing development and expansion of the community clozapine service across the Trust.
* To work collaboratively with colleagues across community and inpatient services to support continuity of care.
* To uphold Trust values, policies, and procedures, including those relating to safeguarding, confidentiality, and risk management.

Person Specification


Experience

* Awareness of safeguarding principles
* Demonstrate an ability to make and sustain professional therapeutic relationships
* Good communication and listening skills
* Ability to work with people in a group and individual basis
* Demonstrate an ability and awareness to maintain personal boundaries
* Able to plan work, time management, completing records and recording observations etc.
* To take leadership and direction from qualified staff.
* To participate in unsupervised tasks and provide accurate feedback to relevant parties
* To demonstrate an ability to provide practical support and physically perform nursing duties.
* To be able to respond promptly to physical and mental health emergencies, be able to think quickly and deal with situations in a calm and sensible manner.
* To be patient and tolerant in challenging situations.
* Knowledge of mental health and a basic understanding of physical health presentations.


Qualifications

* Basic Education
* To complete the Care Certificate within the first 6 months of employment unless already obtained or evidence of equivalent agreed and signed off by the appointing manager at interview. If not, this will be a trainee position until successful completion
* NVQ level 3, QCF Level 3 in Health and Social Care or equivalent qualification.
* Other evidence of training or development relevant to working within mental health settings.

* Mobility throughout the area covered by the trust to meet the requirements of the role


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£24,937 to £26,598 a year pro rata (30 hours per week)
