The Care Co-ordinator role is essential in ensuring the smooth running, and effective delivery of high-quality domiciliary and community-based care services. The Care Co-ordinator is responsible for care planning, staff allocation, rota management and effective communication within the organisation. Care Coordinator Key Responsibilities Care Assessments and Planning Support assessments for new and existing service users. Maintain, update and review personalised care plans. Staff Scheduling and Rota Management Respond to staff absences and ensure continuity. Service Delivery & Operational Support Ensure shifts and visits run smoothly. Support the Registered Manager in daily operations. Quality Assurance and Compliance Conduct spot checks, audits and client reviews. Report incidents and safeguarding concerns. Staff Support and Supervision Provide communication, guidance and updates to care staff. Assist with performance monitoring. On-Call Responsibilities Participate in on-call rota and emergency response. Care Coordinator Person Specification Essential: Previous care coordination or scheduling experience. Strong organisational and communication skills. Desirable: NVQ Level 3. Experience in domiciliary care. Highly organised Manage own work with minimal supervision. Calm under pressure
