Associate Specialist Palliative Care Nurse

Join Our Team: Palliative Care Specialist Opportunity

We are seeking a dedicated professional to provide evidence‑based palliative care, advice, and support to patients diagnosed with cancer or other life‑limiting illnesses, along with their caregivers. In this role you will be accountable for your own professional actions without direct supervision.

The post will also include providing education to registered nurses in care homes to support end‑of‑life care in these facilities.


Key Responsibilities

Plan and deliver education to residential and nursing homes to maximise care to palliative patients.

* Deliver high‑quality palliative care as part of a seven‑day service, collaborating with nursing, medical and AHP colleagues across departmental and organisational boundaries.
* Provide guidance on a variety of clinical and ethical issues, promoting palliative care education among various professional disciplines.
* Support clinical audits within your area of practice, contributing to the process of Clinical Governance for the Trust.


Main duties of the job

We are looking for a dedicated and compassionate Community Palliative Care Nurse to join our team, managing a caseload of patients and their families or caregivers with palliative care needs. You will assess, develop and implement individualised care programmes tailored to the unique needs of each patient, and deliver education to care home staff to support palliative care.


Key Responsibilities

* Work with registered nurses in care homes to educate in palliative and end‑of‑life care
* Manage a caseload of patients and their families or carers with palliative care needs, assessing, developing and implementing comprehensive care programmes.
* Provide expert palliative care advice and support to patients, caregivers and healthcare professionals, utilising your specialist knowledge to manage pain and symptom management, psychosocial needs and ethical dilemmas. Collaborate with other Palliative Care Nurses as necessary.
* Assist patients in achieving self‑care when appropriate and educate caregivers to enable them to provide effective support at home.
* Establish and maintain highly effective communication with patients, caregivers and professionals across health and social care, developing therapeutic relationships to navigate sensitive, distressing and complex issues.
* Act as an advocate for patients, families and caregivers when necessary, ensuring that their needs and wishes are prioritised.
* Work flexibly and during unsocial hours to meet the demands of the service.

Person Specification – Qualifications & Knowledge, Training & Experience

* Registered Nurse with current NMC Pin
* Post‑registration with palliative care or relevant courses
* Registered Mentor
* Demonstrate an understanding of current palliative care issues
* IT literacy
* Degree/degree pathway/diploma in a health‑related subject
* Demonstrate an understanding of research and how to apply research findings in practice
* Demonstrate an understanding of managing change in clinical practice
* 2‑day enhanced communication skills training or equivalent
* Willingness to undertake independent non‑medical prescribing


Skills & Abilities

* Evidence of working with MDT
* Time management
* Ability to travel to different geographical locations across the Trust footprint in line with service needs
* V300 or willingness to undertake independent prescribing
* Management and leadership experience
* Full UK driving licence with willingness to use own car for work purposes
