The Older Adult Community Mental Health Team in Enfield is a friendly, supportive, and dynamic team comprising Social Workers, Occupational Therapists, and Care Co-ordinators. We support older adults with mental health disorders, working both in the community and at Chase Farm Hospital.
Our focus is on transforming lives through a person-centered, strengths-based approach. You will collaborate with individuals, families, carers, and partners to assess needs, plan, implement, and review care actions, promoting independence and social inclusion.
Safeguarding is a priority, and you'll work closely with the Multi-Agency Safeguarding Hub (MASH) to protect adults at risk, conducting s42 Enquiries and Safeguarding Adults Reviews as per the Care Act 2014.
Required knowledge and experience include understanding the Care Act 2014, conducting assessments, managing personal budgets, developing Support Plans, and performing Mental Capacity and Continuing Health Care assessments.
