Young People Leaving Care Support Worker – Preston Area – East Lancs
Position Overview: This outreach role involves driving to meet young people for appointments. The office is based in Preston, covering a broader area including Wigan. A valid UK driving license with business insurance and access to your own car are essential.
Contract Details: Immediate start on a long-term temporary contract, with the possibility of becoming permanent after 12 weeks. The contract will last at least 3 months.
Compensation: £15.50 per hour plus 40p mileage.

Experience & Qualifications:
* Experience supporting young people and their families.
* Full UK Driving License with Business Insurance.
* Experience in similar settings preferred.
* NVQ Level 3 Children & Young People Workforce or equivalent preferred.
Working Hours: 9-5, Monday to Friday, with flexibility to meet the needs of the young people. A current online DBS is required for a quick start.
Job Purpose: To provide support to vulnerable young people and their families, working closely with social workers and other agencies.
Main Duties & Responsibilities:
1. Assist young people's social workers in service delivery, including:
* Providing transport and escorts as per care plans.
* Conducting welcome and induction sessions.
* Assessing and supporting service users' needs.
* Supporting welfare and NHS issues.
* Preparing reports and arranging appointments with partner agencies.
* Maintaining contact with service users and informing them of support groups.
* Supporting move-on processes.
1. Maintain accurate electronic records of service users.
2. Attend meetings and provide mediation as required.
3. Use IT packages effectively to support work.
4. Work collaboratively across multidisciplinary teams.
5. Maintain confidentiality at all times.
6. Assist in policy development and report housing issues.
7. Undertake on-call duties and participate in office reception duties as needed.
8. Support service users on a 1:1 basis or in small groups, including sleep-in shifts.
9. Be flexible to cover all company offices as required.
