What You’ll Be Doing As a Reablement Support Worker, you’ll support people in their own homes—helping people to maintain their independence in: Personal care (washing, dressing, continence support) Meal preparation and medication Encouraging independence through an outcome-focused approach following their reablement pathway Maintaining a safe environment Working alongside families, Social Workers, OT’s and Physiotherapy teams including other professionals to provide a therapeutic service Reporting changes and maintaining care records and peoples progress on their reablement pathway Who We’re Looking For No experience?
