The post holder will be a proactive role model who designs, plans and delivers education and training to enhance the end-of-life care provided by professionals across Calderdale and Huddersfield Foundation Trust (CHFT) and Calderdale community settings including GPs and Care homes. The hours will be between Monday and Friday, working flexibly to cover the needs of the service.
They will work alongside the CHFT End-of-Life Care (EOLC) Facilitator in determining the Palliative and EOLC training and education requirements. The post holder will establish, develop and maintain effective working relationships within the Trust, local hospices, CHFT clinical educators and other EOLC education providers and engage and influence all levels of staff regarding end-of-life care. This will be an important aspect of this role, ensuring CHFT plays an active part in addressing challenges related to palliative and end of life care and promote the standards and expectations as outlined in the ambitions for palliative and end of life care: a national framework for local action 2021 -2026.
The post holder will ensure that CHFT staff are supported to receive the appropriate EOLC training specific to their role to enhance knowledge, confidence and skills in end-of-life care.
In addition, the post holder will facilitate quality and service improvement initiatives to support and develop the provision of end-of-life care.
Main duties of the job
* Plan, design, deliver and evaluate staff training and education courses and events that will attract all professionals involved in the delivery of care at the end of life to provide high quality care.
* The post-holder will support the development and evaluation of the end-of-life care team to improve standards of end of life patients care and promote a seamless service.
* The role requires the ability to work autonomously and independently but within an integrated collaborative team. Follow professional guidance and refer to the End-of-Life Care Facilitator as appropriate.
* Promote outstanding care and work in a way that is patient-centered, involves service users, responds positively to feedback, and promotes learning and continuous improvement.
* Communicate effectively with colleagues on a timely basis to ensure staff and managers remain informed, engaged and own the work. This will include providing and receiving sometimes highly complex information which may be sensitive or contentious, where persuasive, motivational and negotiating or empathetic and re-assurance skills are required.
