Are you a health and social care practitioner with experience of supporting people living with Long Term physical and mental Health Conditions to self-manage? Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities? The post holder will: Deliver holistic one-to-one and group based self-management support to young people and adults living with long term health condition(s) referred from Airlie Medical Practice and Scoonie Medical Practice Primary Care teams Practice persistent engagement to help people who struggle to receive support. Deliver a peer development programme by supporting local peer volunteers to set up new self management supports in the community. Use a monitoring, evaluation and reporting framework to learn and develop practice. Develop strong working relationships with the teams working within Airlie Medical Practice and Scoonie Medical Practice and develop and maintain links with community supports they signpost people to.
