Overview
Oak Tree Ward is a 17‑bedded Older Adult Mental Health Inpatient unit. We are a service for those mainly over the age of 70 (although we are ageless), those living with cognitive impairment, or those with a high frailty score over 6. We are an assessment ward accepting people for admission from home, care homes and acute hospitals.
Conditions and Team
We support service users with:
* Anxiety
* Bi‑polar disorder
* Depression
* Dementia
* Parkinson's disease
* Personality disorder
* Schizophrenia
Our multidisciplinary team includes consultant psychiatrists, junior doctors, ward managers, mental health nurses, dual‑trained nurses, nurse associates, healthcare support workers, occupational therapists and physiotherapists.
Role and Responsibilities
This job role involves working under the supervision of registered nurses to support our clients to achieve their day‑to‑day needs:
* Washing and dressing – ensure all patients are offered support with a shower, changing their clothes, maintaining oral hygiene and supporting as needed.
* Supporting patients with their toileting needs throughout the day to prevent incontinence and skin breakdown.
* Monitoring patient finger and toe nails and offering nail care when needed.
* Maintaining adequate dietary intake by monitoring all patients during meals, documenting and reporting any concerns around refusal and supporting patients to eat their meal when required.
* Undertaking patient vital signs, being aware of NEWS2 and the correct ranges, and escalating concerns to nursing and medical teams.
* Observing patients on different levels of observation, including constant supervision.
* Maintaining documentation such as observations, fluid chart, NEWS2 scores, stool chart and sleep chart.
* Maintaining legible documentation and being competent with IT systems.
* Maintaining mandatory training.
Training and Development
* Opportunities to develop your skills through mandatory training, ECG training and venepuncture training.
* Opportunity to complete a Care Certificate if not already done.
Shift Patterns
* Day shifts – 07:30 am to 08:00 pm
* Early shift – 07:30 am to 03:30 pm
* Late shift – 12:00 pm to 08:00 pm
* Night shift – 07:30 pm to 08:00 am
