Advanced Practitioner – Qualified Social Worker
Specialism: Care Leaver Development
South Yorkshire | £37 per hour | Part-time – 2.5 days per week
Are you a passionate and experienced Social Worker ready to make a lasting difference in the lives of care-experienced young people? We’re looking for a dynamic Advanced Practitioner to join a dedicated Care Leaver Development Team in South Yorkshire. This is an exciting opportunity to be part of a forward-thinking service that supports young people leaving care to build strong foundations for a safer, brighter future.
The Role
* Lead meaningful, direct work with care-experienced young people and their families.
* Complete in-depth assessments and care planning, intervening early to ensure better long-term outcomes.
* Work alongside key partners including police, health services, and early help teams.
* Contribute to safeguarding work, including Section 47 enquiries, Initial Child Protection Conferences, and permanency planning.
* Help bridge the gap between social care and local community support by promoting sustainable, positive change.
* Offer guidance and support to students and unqualified staff, helping shape the next generation of social care professionals.
What You’ll Bring
* A qualified Social Worker, registered with Social Work England.
* Experienced in working with vulnerable children and families – including care leavers.
* Confident in risk assessment and care planning.
* Knowledgeable of child protection legislation and best practice.
* Skilled in building strong, trust-based relationships with young people and families.
* A collaborative team player, comfortable working across disciplines and agencies.
* Emotionally intelligent, flexible, and resilient – with a drive for continuous improvement.
* Minimum 4 years post-qualified experience required.
