An exciting opportunity has arisen for a Band 5 Rotational Physiotherapist to join the East Coast Community Healthcare Physiotherapy Rotation, covering a number of specialist services within the community.

Currently the rotation covers Primary Care Homes, Integrated Community and Neurology, Musculoskeletal and Intermediate Care.

The Integrated Community and Neurology Team - The team supports patients with long-term neurological conditions across the Great Yarmouth and Waveney area, and consists of specialist nurses, physiotherapists, occupational therapists and rehabilitation assistant practitioners. Assessment and intervention is completed in clinic settings as well as in patients homes and care homes. We provide timely assessments and treatment to facilitate and improve patients quality of life and maximise independence. You will take responsibility for a designated caseload of patients, supporting them throughout their journey from initial diagnosis, management and palliative care.

Primary Care Homes - The Team support patients in the community with complex care needs who require specialist physiotherapy input to aid in function and mobility. Supporting patient to achieve optimal independence. You will be working as part of the MDT teams which include, nurses, occupational therapists, paramedics, rehabilitation support works, assistants practitioners, therapy assistants, pharmacy support, health care assistants and community matrons.


Main duties of the job

MSK Team - Provides the assessment and treatment of patients with musculoskeletal conditions. Utilising health coaching techniques to empower self management.

Intermediate Care Team - Provides care to patients with highly complex needs within a ward-based setting however there may be a requirement to work across a wider community setting. The emphasis of the service will be reablement of patients in order to promote; Faster recovery from illness, prevention of unnecessary acute hospital admissions and premature admission to long term residential care, support for a timely discharge from hospital and maximise independence.

The successful applicant will be able to demonstrate a high level of clinical expertise, as well as excellent communication and organisational skills.

We provide a structured supervision program, including clinical support and regular in-service training. In return, we expect applicants to show a commitment to Continued Professional Development and evidence based practice.

Job responsibilities

CLINICAL:

* To be professionallyand legally responsible and accountable for all aspects of your own workincluding the management of patients in your care. To ensure a high standard ofclinical care for the patients under your management and support others to dolikewise.
* To interpret andanalyse clinical and non-clinical facts to form an accurate diagnosis andprognosis in a wide range of conditions, to recommend the best course ofintervention and to develop comprehensive management strategies for the patient'spresenting condition.
* To undertakecomprehensive assessment of patients, using investigative and analytical skillsand to formulate individualised management and treatment plans, using clinical reasoning and utilising awide range of treatment skills and options to formulate a specialised programmeof care.
* To accept clinicalresponsibility for a designated caseload of patients and to organise thiseffectively and efficiently with regard to clinical priorities and use of time.
* To demonstrate highlydeveloped precise dexterity, co-ordination and palpatory senses for assessmentand manual treatment of patients.
* To providespontaneous and planned specialist advice, teaching, and instruction torelatives, carers and other professions to promote understanding of the aims ofphysiotherapy and to ensure a consistent approach to patient care.
* To be responsible forequipment used in carrying out physiotherapy duties and to adhere to anyrelevant policies, including competence to use equipment and ensure safe use ofequipment by others through teaching, training and supervision of practice.
* To supervise band 3and band 4 team members when appropriate.
* To provide clinicalsupervision for physiotherapy students on placement and to explain the role ofphysiotherapy to departmental visitors.
* To assist more seniorstaff in the training of students and physiotherapy assistants.
* To communicateeffectively with patients and carers to maximise the rehabilitation potentialand to ensure understanding of the condition. Communication skills ofpersuasion, motivation and explanation will be used with a variety of patients.
* Communicateeffectively with all members of the physiotherapy team, GPs, consultants andother professions to ensure a collaborative approach to the management of thepatient's condition.
* To assess capacity,gain valid informed consent and have the ability to work within the legalframework with patients who lack capacity to consent to treatment.

PROFESSIONAL:

* To maintain ownclinical professional development (CPD) by keeping abreast of any new trendsand developments and incorporate them as necessary into your work.
* To be an activemember of the in-service training programme by attendance at training sessionsin-house, tutorials and by attending external courses and practicing reflectivepractice.
* To maintain accurate,comprehensive and up to date documentation in line with legal, departmental andECCH requirements, communicating assessment and treatment results to theappropriate disciplines in the form of reports and letters.
* To participate instaff appraisal scheme and personal development plan (PDP).
* To undertake themeasurement and evaluation of your work and current practices through the useof evidence based practice (EBP) projects, audit and outcome measures, eitherindividually or with other clinical staff and the clinical lead forphysiotherapy. Make recommendations for change.
* Demonstrate a soundunderstanding of Clinical Governance and Risk Management and apply to the worksituation.

ORGANISATIONAL:

* To be responsible fora designated area of work as agreed with the team leader and to plan andorganise efficiently and effectively with regard to patient management and useof time.
* To decide prioritiesfor a work area, balancing other patient related and professional demands.Ensure that these remain in accordance with those of the service as a whole.
* To be activelyinvolved in the collection of appropriate data and statistics for use in thephysiotherapy service.
* To be aware of Healthand Safety aspects of your work and implement any policies, which may berequired to improve the safety of your work area, including prompt recordingand reporting of accidents to senior staff and ensuring that equipment use issafe.
* Ensure theimplementation of service policies and any service development changes.
* To comply with theorganisational and service policies and procedures and to be involved in thereviewing and updating as appropriate.
* To undertake anyduties that might be considered appropriate by a senior clinician, Clinicallead/coordinator and professional Lead.

* Previous experience as a Band 5 physiotherapist

* Physical capacity to deal with moderate physical effort on a daily basis. Including; transporting equipment, mobilising patients, supporting weight of patients limbs for mobilising and other treatment techniques
* Physical and mental capacity to deal sensitively with patients who may have high levels of anxiety and aggression
* Capacity to concentrate for considerable periods of time during assessment and management of patients
* Able to work collaboratively within a team
* Continuing professional Development
* Able to maintain judgement under pressure
* Ability to work in unpleasant working conditions for example; poor body hygiene of patients, contact with body fluids and unhygienic domestic conditions.
* Willingness and ability to work across different sites and travel to alternative sites and across the community as required

* Degree in Physiotherapy
