What You'll Do
1. Craft Personalized Care Plans: Collaborate with residents, their families, and healthcare professionals to design and implement care plans tailored to each individual.
2. Ensure Accuracy: Observe and document daily interactions and conditions accurately, ensuring all records are clear, precise, and compliant with legal standards.
3. Lead with Confidence: Organize and guide your team at the start of each shift, ensuring everyone understands their roles in delivering excellent care.
4. Facilitate Seamless Transitions: Oversee shift handovers to ensure smooth communication and continuity of care.
5. Monitor Health: Conduct routine observations and manage specific health needs with attention to detail and safety.
6. Administer Medications: Safely manage and administer medications following MAR sheets and protocols.
7. Build Relationships: Foster trusting relationships with residents, families, and healthcare professionals, addressing concerns with empathy and urgency.
8. Promote Independence: Support residents in daily activities, encouraging autonomy while providing assistance as needed.
9. Act on Feedback: Respond to team input to continuously improve resident care and well-being.
10. Champion Respect: Uphold residents' dignity and individuality, advocating for their needs and preferences.
11. Encourage Engagement: Facilitate social activities and interactions to enrich residents' lives and foster a vibrant community.
12. Prioritize Safety: Maintain vigilance regarding health and safety standards to ensure a secure environment.
13. Support Nutrition: Monitor residents' nutrition and hydration, especially those at risk of weight changes or dehydration.
14. Respond to Emergencies: Be prepared to act swiftly in emergencies, utilizing the on-call system and reflecting on practices to improve care quality.
About You
* You are a Registered General Nurse (RGN) or Registered Mental Health Nurse (RMN) with an active NMC PIN.
* Care home experience is a plus, but your passion for care and dedication to residents' well-being are what truly matter.
* You have a natural ability to connect with residents, families, and colleagues, bringing warmth, understanding, and patience to every interaction.
* Excellent communication skills are essential—whether discussing residents' needs, updating families, or guiding your team, you do so with clarity, empathy, and respect.
