Community Support Services Coordinator

Base pay: Hybrid – Home with regular travel within Western Isles
Roles and Responsibilities
* Play a vital role in delivering Community Support Services across the Western Isles for people living with chest, heart, stroke conditions, and Long Covid.
* Lead a high‑quality, person‑centred service that empowers individuals to build confidence, manage their condition, and work towards their own goals.
* Recruit, retain, and line‑manage volunteers who provide goal‑oriented, time‑limited support, helping people take positive steps towards greater independence and self‑management.
* Develop strong local connections and maintain a detailed community map to support effective signposting, ensuring people can access the right support at the right time.
* Network with referrers in the local area and recruit a strong team of local volunteers within their community.
* Provide community support on a daily basis in either a group or 1‑to‑1 setting, and travel across Scotland for training, meetings, and conferences as required.
* Support flexible recruitment through Working Families and offer flexible working arrangements.
