As Mental Health Rehabilitation Co-ordinator the main duties will be:
* To support the multi-disciplinary team by determining recovery orientated goal plans including identifying recovery focussed activities and fostering a culture of hope
* Develop, implement and evaluate care plans in conjunction with service users, carers and other professionals which reflect individual needs
* Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs
* To work alongside a number of service users on a one to one or group basis, potentially within the individuals own home, or support junior staff, providers and VCSE organisations to carry out this work effectively
