DSR and Learning Disabilities and Autism Commissioning Manager
Job description: We are seeking an enthusiastic and passionate individual to manage commissioning services for adults and children with learning disabilities and/or autism. The role involves preventing hospital admission and supporting discharge planning, as well as commissioning community‑based services to meet individual needs.
Main duties of the job
* Managing the referral and review processes for the Dynamic Support Register.
* Undertaking Care (Education) and Treatment Reviews.
* Providing case management support to autistic people and those with learning disabilities who are at risk of hospital admission or currently receiving inpatient treatment.
Job responsibilities
* Manage own delegated portfolio across the LDA Commissioning team with line manager support.
* Provide management and leadership to support staff working directly and indirectly with the DSR process.
* Work with people with learning disabilities and/or autism to maximise their independence with the appropriate support and services.
* Plan, promote and deliver services.
* Ensure the service delivery, both directly and commissioned, is in line with national policy and guidance concerning the Dynamic Support Register (DSR), Care (Education) and Treatment Reviews (C(E)TRs) and Case Management in a person‑centred and individualised approach.
The service operates Monday to Friday, from 9 am to 5 pm. We understand the importance of work/life balance and welcome discussions around flexible working arrangements as required.
Person Specification
QUALIFICATIONS
* Professional Health or Social Care qualification and current registration.
* Evidence of a commitment to lifelong learning.
KNOWLEDGE/EXPERIENCE
* Significant experience working with people with learning disabilities and/or autism needs in a paid or voluntary capacity within health/social care/teaching settings.
* Knowledge and experience of resources, systems and processes available to support admission avoidance, discharge planning and community support.
* Experience and knowledge of running Multi‑Disciplinary Meetings (MDT).
* Good knowledge and experience or organisational cultures and multi‑agency working.
* Experience of assessing risk in accordance with set criteria and identifying ways of risk mitigation.
* Experience of supervising others.
* Experience of working as part of a team.
