Overview
Advanced Pharmacist Elderly Care, Rehabilitation and Frailty role at Northamptonshire Healthcare NHS Foundation Trust. NHFT is looking for an experienced clinical pharmacist to lead and support the delivery of a clinical pharmacy service to their community inpatient beds. The post holder will act as clinical lead for pharmacy services to Community Beds within NHFT. Our community hospitals offer inpatient specialist services for adult patients who need 24-hour inpatient care, focusing on sub-acute medicine with medical, physical and neurological needs.
The role requires independence to work on your own and within the pharmacy team, with travel between sites across Northamptonshire.
Responsibilities
* Lead, deliver and evaluate clinical pharmacy services to patients within Community Beds
* Provide a clinical pharmacy service to Community Beds inpatient areas, working with other pharmacists and pharmacy technicians
* Offer expert advice on pharmaceutical matters, undertake risk management and ensure compliance with medicines legislation and local policy
* Facilitate and support the efficient and cost-effective use of medicines
* Collaborate with staff within the Pharmacy department and across the Trust to enable the Trust objectives
* Cascade Medicines Management Committee decisions to Community Beds team
* Contribute to Trust compliance with Controlled Drugs, Pharmacy procedures, Medicines Management standards, Clinical Governance and Risk Management
Requirements / Qualifications
* Experience as a clinical pharmacist with leadership capabilities
* Ability to work independently and within a team
* Willingness to travel between Trust sites across Northamptonshire
