City Community Team are seeking 2 Community Nurses based at Five Rivers Health and Wellbeing Centre. We provide complex care in the community setting, including caring for vulnerable individuals in challenging environments.
Job Summary
Experience in NHS community patient care is essential, along with up-to-date clinical professional development. Candidates should have excellent clinical skills, strong communication, and the ability to work under pressure. We seek enthusiastic, motivated, and flexible individuals.
Our team cares for patients registered with South Wiltshire GPs, collaborating with multidisciplinary team members like Occupational Therapists, Physiotherapists, and Specialist Practitioners. Shift patterns include 7-3, 8.30-4.30, and 2-10pm.

Responsibilities
