Job summary
As a Consultant in Acute/General Medicine with Shropshire Community Health NHS Trust, you will play a pivotal role in delivering high-quality, patient-centred care within our innovative Virtual Ward Service. You will provide clinical leadership, oversee the management of patients with acute and long-term conditions, and work collaboratively with a multidisciplinary team to deliver care in community settings.
This role combines direct clinical care, service development, and education, offering the opportunity to shape the future of acute medical services outside the traditional hospital environment. You will be instrumental in supporting the Trust's vision to provide accessible, holistic care closer to patients' homes, while also contributing to quality improvement and staff development initiatives.

Main duties of the job
* Provide clinical leadership for the Virtual Ward, working with a skilled multi-professional team.
* Deliver expert care for patients with acute and long-term conditions, both remotely and face-to-face.
* Support service development, quality improvement, and staff education.
* Collaborate with acute and community partners to deliver seamless care.
