Overview
Nottinghamshire Healthcare Trust currently have a vacancy within the In-Reach department, which sits within the Urgent Care Group. The In-Reach Team is a small team consisting of a Team Leader, two Band 7 Clinical Leads and four Band 4 Practitioners. Cover is provided at Sherwood Oaks and Highbury Hospital in Bulwell. We currently have 1 position available that will cover Sherwood Oaks, although cover may also need to be provided across sites. This position is a full-time permanent post. Working hours are Monday to Friday, 9am to 5pm.
Responsibilities
* Undertake an assessment of housing and social needs of individuals with mental health difficulties who are in-patients on mental health wards within the county area of Nottinghamshire Healthcare NHS Foundation Trust.
* Act as a lead in relation to addressing identified housing and social difficulties.
* Pro-actively engage within bed management meetings across the Trust, working in collaboration with statutory and non-statutory organisations to reduce hospital in-patient stays and contribute to the reduction of delayed transfer of care (DTOC).
* Act as a resource for social and housing difficulties and related issues across the acute community services.
* Collaborate with other statutory, non-statutory and voluntary teams, agencies and organisations; demonstrate high level communication skills.
* Be proactive in attending wards to undertake full assessments of needs on a face-to-face basis.
* Actively engage the service user with appropriate statutory and non-statutory organisations.
* Engage with family / carers where appropriate in relation to social difficulties if agreed.
* Attend county-wide DTOC / bed management meetings and use problem-solving techniques to engage with service users where there are discharge difficulties.
Requirements
• Confident experienced clinician with an interest in housing and social needs within mental health services.
Location and Hours
Location: Sherwood Oaks (with potential coverage at other sites).
Hours: Monday to Friday, 9am–5pm. Full-time permanent role.
