Assistant Patient Safety Officer – 37.5 hours – Fixed 12 months
The post holder will be based within the Patient Safety Team and will be line‑managed by the Patient Safety Officers. The role is central to delivering incident review and management, supporting the Site Safety Huddle and Incident Review Group (IRG) and ensuring that the statutory Duty of Candour requirement is met.
Responsibilities
* Support the Site Safety Huddle and capture actions accurately.
* Support the Incident Review Group (IRG) and Patient Serious Incident Review Group (PSIRG), including preparing agendas, confirming attendees and following up decisions and outcomes.
* Provide patient and family support via the Duty of Candour process, including attending and supporting family meetings.
* Develop, implement and deliver efficient and robust administrative processes to support the work of the Patient Safety Officers.
* Ensure that all relevant policies and procedures, such as information governance and record keeping, are adhered to.
* Line‑manage, train and supervise the Patient Safety Support Officers, recruiting and training new staff as required.
* Communicate with patients and relatives, both verbally and in writing, about patient safety incidents and learning responses.
* Ensure statutory requirements relating to Duty of Candour are met.
* Arrange and support meetings with senior Trust members, setting agendas, preparing for meetings, taking accurate minutes and updating action logs.
* Provide guidance and advice on Patient Safety learning responses in line with PSIRF.
