A new and exciting locum opportunity has arisen within a fast paced Hospital discharge team in the Halton, Merseyside area.
The role will be based within a fast paced Hospital Discharge Team. The team is made up of a team of dedicated health and social care staff, working together to provide holistic assessment and support plans to ensure the best possible outcomes..

Responsibilities:
* You will carry out assessments under the Care Act 2014 to determine eligibility need
* Maximise rehabilitation services such as reablement & intermediate care where appropriate
* Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life.
* Work with patients and families addressing the impact of illness and treatment
To be considered for the Social Worker role:
* 1-3 years experience of working within a similar setting
* Diploma/Degree in Social Work
* SW England registration
* Experience of working under the care act and mental capacity act
* Hospital experience is an advantage
