Optimising Medication Therapy and Promoting Safe Practice

The Clinical Pharmacist plays a crucial role in ensuring safe and effective medication use within the hospital. This involves direct patient care collaboration with physicians, nurses, and other healthcare professionals to improve treatment outcomes and minimise medication-related problems.

"

Responsibilities:

* Deliver high-quality clinical care and advice where needed
* Offer services to various departments, including pre-assessment clinics, outpatient departments, wards, and theatres
* Participate in ward rounds and multidisciplinary team meetings
* Educate patients on proper medication use
* Conduct medication reconciliation and drug utilisation reviews
* Document clinical interventions and outcomes
* Provide drug information and training to healthcare staff and patients
* Ensure compliance with hospital policies, national regulations, and accreditation standards
* Participate in quality improvement initiatives, audits, and medication safety programs
* Report adverse drug reactions and medication errors
"

Requirements:

To be successful in this role, you will require strong knowledge of pharmacology, pharmacokinetics, and disease management. Excellent communication and interpersonal skills are also essential, as well as attention to detail and problem-solving abilities.
