What you will do:
1. Implement and manage pathway planning for children and young people in the care system, ensuring tailored and supportive plans.
2. Conduct regular keeping in touch visits and maintain accurate records of these interactions.
3. Use Liquid Logic to update and manage case records, ensuring currency and accessibility of information.
4. Attend monthly case supervision sessions, engaging in reflective practices to enhance service provision.
5. Collaborate closely with team members and stakeholders, including participation in reflective group supervision.
6. Assess risk and coordinate support around care leavers, working to mitigate potential challenges.
7. Support team managers by providing insights and expertise to advance pathway planning for separated young people.
8. Drive to various locations as needed, requiring a full driving license and access to a vehicle with business insurance.
