JOB SUMMARY
We are seeking a compassionate, motivated and reliable Reablement Assistant to join our team in supporting adults across Liverpool to maintain independence and enhance their quality of life. You will work directly with individuals in their own homes, helping them develop and retain essential daily living skills while promoting dignity, choice, and wellbeing.
JOB DESCRIPTION
The duties and responsibilities of the role will include:
Support the physical, social, and emotional needs of service users in line with personalised care plans.
• Promote independence by helping individuals develop daily living skills and confidence.
• Assist service users with tasks identified in their support plans, using appropriate equipment when required.
• Work in partnership with allied health and social care professionals to minimise risk and enable service users to remain safely at home.
• Implement person centred support plans and contribute to assessments and reviews.
• Champion good care practice, ensuring dignity, respect, empowerment and cultural sensitivity at all times.
• Maintain accurate records and report any significant changes in service users’ condition or circumstances to your Line Manager.
• Participate in supervision sessions and work collaboratively as part of an integrated team.
• Support service users to engage in activities, hobbies, and meaningful daily routines.
• Be flexible and responsive to the evolving needs of the service.
• Work in alignment with City Council policies, including safeguarding, equal opportunities, and anti-discriminatory practice.
• Ensure all duties are carried out in accordance with health and safety legislation and the City Council’s safety plan. THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

• Strong communication and interpersonal skills, with the ability to build positive relationships.
• The ability to work collaboratively as part of a multi-disciplinary team as well as independently.
• Good organisational skills and the ability to follow structured care and support plans.
• A commitment to promoting independence, dignity, choice and respect at all times.
• The ability to recognise and report changes in service users’ needs or circumstances.
• Reliability, flexibility, and a proactive approach to responding to changing service demands.
• An understanding of safeguarding, equality, diversity and anti-discriminatory practice.
• Basic IT and record-keeping skills to maintain accurate documentation.
• Previous experience in reablement, home care, support work or a similar care setting (desirable).
