In addition to being an expert in your field with extended competencies such as vacuum assisted closure and syringe driver management, you will need to demonstrate excellent communication skills to elicit an accurate, holistic assessment of patient needs as part of a case management approach. We offer training and support for a Health Coaching approach to enhance patient engagement, particularly for those with long-term conditions.
As part of the role, you will work within a dedicated referral centre and will possess well-developed critical thinking and negotiating skills to support decision-making and triage processes. You will be responsible for managing a defined caseload of patients, with delegated responsibilities to your Band 5 nursing team, to ensure continuity of care within the nursing model. Prior experience with electronic patient record systems (e.g., SystmOne) is advantageous; full training and support will be provided.
You will be aware of the importance of collaborative working and have some understanding of the services and other professions within the Gloucestershire Health and Care NHS Foundation Trust, including Therapy colleagues within the ICT (Integrated Community Team), GP surgeries, Rapid Response, and Complex Care at Home. This ensures the right care is delivered at the right time by the right professional for patients.
