Main duties of the job
* Deliver core nursing services including:
* Cervical cytology
* Chronic disease management (e.g. Asthma, COPD, Diabetes)
* Wound care
* Immunisations (adults & children)
* Contraceptive advice
* Conduct patient assessments, develop and evaluate care plans
* Home visits when required
* Promote health education and lifestyle advice
* Make appropriate referrals to internal/external health professionals
* Support QOF achievement and other clinical targets
* Participate in clinical audits and implement improvements
* Maintain accurate, timely documentation using EMIS Web
* Monitor and order medical supplies; ensure appropriate storage
* Support maintenance and servicing of clinical equipment
* Ensure clinical areas are well-stocked and maintained
* Assist with recall and follow-up systems for patient care
* Build effective working relationships across the multidisciplinary team
* Attend internal/external meetings relevant to the role
