Job Summary
A vacancy has arisen in our thriving Primary Care Network covering three GP practices in March and one in Ramsey, Cambridgeshire for an Advanced Nurse Practitioner. We are looking for an enthusiastic and motivated person to join our team. We pride ourselves on teamwork and offer a high standard of support and guidance to all our staff.
Hours are 37.5 hours per week but are negotiable to the right candidate.
The post holder must possess a valid and current NMC registration and maintain current membership of a professional organisation such as the Royal College of Nursing. An enhanced DBS check will be required.
Job Aim
To work independently or as part of the Duty Team to triage, assess, plan and treat emergency or same-day presentations, ensuring high quality, accessible, evidence-based, and clinically effective care to the patients of Fenland PCN. To be involved in the development of the Nurse Practitioner role within the PCN.
The post holder will be an experienced nurse with Level 7 qualifications, including advanced assessment, prescribing, and ideally a Masters degree in Advanced Clinical Practice or be willing to work towards this. They will act within their professional boundaries to work autonomously, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, in face-to-face and telephone appointments.
Main Duties of the Job
1. Assess, diagnose, plan, implement, and evaluate treatment/interventions.
2. Examine and assess patient needs from a physiological and psychological perspective.
3. Proactively identify, diagnose, and initiate treatment plans for patients.
4. Prescribe and review medication.
5. Create referrals and/or diagnostic interventions as appropriate.
6. Interpret and evaluate diagnostic test results.
7. Work with patients to support compliance with and adherence to prescribed treatments.
8. Ability to carry out home visits to housebound patients and those in care homes.
9. Assess, identify, and refer patients presenting with mental health needs.
10. Participate in team discussions to improve patient care.

Job Responsibilities
1. Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis including those with complex needs, demonstrating safe clinical decision-making skills.

Qualifications
Essential
* Valid and current NMC registration and membership of a professional organisation such as the Royal College of Nursing.
* Enhanced DBS check.
* Full UK driving license.
Desirable
* Qualified in Phlebotomy.
* Independent prescriber.
* Experience in primary care.
