We have an exciting opportunity to join the award-winning rough sleeper team. We are looking for an experienced mental health nurse to provide clinical knowledge and interventions, advice and support to rough sleepers across Shropshire and to the wider RESET service. The post holder will undertake rapid assessment and support to adults 18+ who are homeless or rough sleeping. The post holder may also be called upon to provide mental health support to adults who are housed and supported by the local authority but whose tenancy is in jeopardy due to a decline in their mental health.
The post holder will be expected to participate in the multi‑agency RESET meetings to provide guidance, signposting and clinical knowledge to the MDT. The post holder will be responsible for providing leadership to deliver evidence‑based care and interventions and for professional nursing leadership within the pathway team.
Main duties of the job
Take responsibility for the assessment of a complex cohort, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.
* To provide appropriate advice to members of RESET, local authority staff, third sector staff, multi‑disciplinary and Management Groups.
* To monitor and evaluate practice based on contemporary and current trends and initiate changes in the light of these findings.
* To promote, encourage and develop integration between MPFT, RESET and third sector organisations.
* Facilitate effective early assessments for individuals identified through the rough sleeper task force or referred from housing or third sector colleagues.
* To aid discharge planning from acute inpatient units and local hospitals PRH and RSH.
* Encourage and facilitate multi‑disciplinary and multi‑agency working.
Person specification
Qualification
* Evidence of professional registration and relevant post‑registration experience.
* Qualification or training within dual diagnosis or substance misuse.
Experience
* Substantial experience of community nursing care planning and risk assessment.
* Experience of working with clients who experience complex mental, physical and social needs.
* Experience of leadership roles within a team, including creating new roles or making changes to existing workplace.
* Experience working with homeless individuals, support services or charities.
* Experience liaising between multiple teams and disciplines from different services.
