What You'll Do: Provide initial assessment and support to new and known service users. Offer expert guidance and information to healthcare professionals. Detailed job description and main responsibilities In this role, you will play a vital part in providing holistic, patient-centered care. You'll conduct comprehensive assessments, considering all aspects of a patient's well-being - mental, physical, social, cultural, and more. By working collaboratively with patients, their families, and other healthcare professionals, you'll develop personalized care plans that address their unique needs. Person specification Experience Essential criteria Registered on NMC One year experience at Band 5 or above in hospital or community health care Evidence of continued professional development, appropriate to role Clear understanding of CPA and care coordination role Desirable criteria Post-registration certificate ENB 998.
