We are looking for Mental Health Outreach Support Worker to join our dedicated team, working directly in the community to support individuals in their own homes and local environments.
What you'll be doing:
* Providing 1:1 outreach support to people living with mental health needs
* Encouraging independence, confidence, and positive daily living skills
* Supporting with community engagement, appointments, and practical tasks
* Promoting recovery-focused and person-centred support at all times
