Were looking for a passionate and skilled Social Worker to join our Transitions Service, supporting young people aged 1725 as they move from childrens services into adult social care. This is a rewarding opportunity to make a real difference at a crucial stage in their lives.
About the role
As a Social Worker in our Transitions team, you will:
* Carry out strengths-based, person-centred Care Act assessments.
* Be the named worker for young people transferring into adult services.
* Build strong, trusting relationships with young people and their families while working alongside childrens social care.
* Develop support plans that promote independence and ensure eligible needs are met.
* Review plans regularly, ensuring least-restrictive, outcome-focused practice.
* Work collaboratively with professionals across directorates, partners and external agencies.
* Identify and manage risks, safeguarding young people and vulnerable adults.
* Keep accurate, timely records on our social care systems.
* Provide guidance and informal supervision to colleagues and contribute to the development of the wider team.
This role plays a key part in supporting young people to achieve their potential and enter adulthood with the right care in place.
About you
You will be a Qualified Social Worker with SWE registration and experience supporting young people or adults with diverse and complex needs. Youll bring:
* Strong understanding of relevant social care legislation and safeguarding practice.
* Experience carrying out assessments and managing complex caseloads.
* Excellent communication and interpersonal skills.
* Ability to work collaboratively and build effective partnerships.
* Strong organisational skills, accurate record keeping and confidence using IT systems.
* A reflective, proactive approach with commitment to your own professional development.
Experience working with people with learning disabilities, autism, mental health needs or SEND is desirable but not essential.
