The post holder is responsible for assessing care needs and developing, implementing, and evaluating care programmes for Adults and Older Adults with mental health issues referred to the Crisis Resolution Home Treatment Team.
The post holder reports daily to the Clinical Lead but is ultimately accountable to the Matron for the overall service. They will work in a multidisciplinary team, collaborating closely with wards and Primary Health Care professionals. They are also responsible for maintaining care standards within the Crisis Resolution Home Treatment service and ensuring an integrated, collaborative approach to client care.

Key responsibilities include:
* Initial assessment of clients for suitability for the service and liaising with referral sources to meet client needs.
* Conducting clinical risk assessments and ongoing monitoring of mental state to evaluate risks to clients and others.
