Senior Specialist CHC Social Worker – Specialist in Continuing Healthcare (CHC)

Are you ready to embark on a rewarding short-term venture? We have an exciting opportunity for a Senior Specialist CHC Social Worker position based in picturesque Chatham, Kent. Over the course of 3 months, you’ll be working full time in the vibrant field of Continuing Healthcare (CHC), earning £40 per hour.
As a Senior Specialist CHC Assessor, you will lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare funding. You’ll also manage complex cases, including disputes and joint funding solutions, while supporting individuals and families through the assessment process with empathy and professionalism. You will work closely with internal teams, care providers, and local authority partners to ensure appropriate care packages are in place—whether or not individuals meet CHC criteria. Your role will also include conducting Section 117 reviews and contributing to service improvement initiatives.
What you will do:
* Lead and coordinate CHC assessments and reviews, both virtual and face-to-face.
* Manage a clinical caseload from initial assessment to resolution.
* Collaborate with stakeholders to arrange suitable care packages and placements.
* Provide specialist advice on CHC, S117, and joint funding pathways.
* Maintain accurate records and support data reporting and governance.
* Contribute to performance targets and service development.
* Support training, induction, and continuous improvement efforts.

What We’re Looking For:
* Registered with NMC or SWE.
* Substantial post-qualification experience in nursing or social work.
* Strong knowledge of CHC, MCA, and consent processes.
* Excellent communication, report writing, and IT skills.
* Ability to work independently and manage a dynamic caseload.
* Full driving licence and access to transport.
* Experience with Section 117 aftercare is desirable.
* Background in mental health, learning disabilities, or older adult services is a bonus.
