Oxleas NHS Foundation Trust is recruiting Associate Mental Health Workers to join our new Psychosis Pathway Care Teams. This is an exciting opportunity to work within a team-based, recovery-focused model, supporting people with psychosis to achieve stability, independence and improved quality of life. You will be part of a supportive multidisciplinary Care Team, sharing responsibility for a caseload and delivering practical, therapeutic and social interventions in the community. We are looking for motivated, compassionate individuals with experience in health or social care who are passionate about working with people with severe mental illness. Posts are available across Bexley, Bromley and Greenwich, with opportunities for development, supervision and training.


Responsibilities

* Support service users with psychosis as part of a shared care team caseload
* Deliver practical, recovery-focused interventions under supervision
* Promote engagement, social inclusion and independence
* Support physical health monitoring and wellbeing
* Contribute to risk management, care planning and reviews
* Maintain high-quality clinical records using Trust systems
* Work flexibly across community settings in line with service need
* Actively participate in supervision, training and reflective practice
* Undertake a range of duties which involve direct patient care, under supervision following competency assessment
* Assist in the coordination of care delivery for a case load of service users in the community with mental health problems including those with severe and enduring mental illness under the supervision of a registered professional, in close collaboration with the multidisciplinary team, service users and other community agencies.
* Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit/community setting.
* Have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all service users
* Actively participate in the care and rehabilitation of service users as per the plan of care determined by registered practitioners
* Maintain clear, timely, comprehensive and contemporaneous written clinical records on all patients under your care in accordance with Trust Records Management Policies (2010).
* Utilising IT skills and knowledge to access and input clinical information and data to the Trust electronic database (RiO) and Local Authority IT system to record service user’s information.
* Using the Care Programme Approach (or equivalent framework) to assess service user’s health and social care needs.
* Contribute towards the delivery of care using the recovery approach to provide therapeutic interventions under supervision by means of assessment and ongoing treatment in various settings. Design care programmes in line with risk assessment and psychosocial and occupational needs.
* Utilise designated recovery tool with outcomes focusing on care planning and ensuring clients understand they will be worked with in a collaborative manner to engage in social inclusion activities such as seeking employment, education and independent living.
* Act in accordance with the direction of an appropriate practitioner (e.g. nurse, doctor or pharmacist) to administer medication as and when required following competency assessment and supervision.
* To be conversant with factors that cause mental illness and preferred treatment methods.
* Support the assessment and management of patient care needs through initial data collection, monitoring of patients progress, feedback, discussion, and reports and in care reviews with the clinical team.
* Provide information relating to health promotion and associated activities to improve the well-being of service users.
* Take part in psychosocial interventions i.e. CBT, psycho-education or family work as per service user care plan.
* Provide patients and their carers with a service that is culturally sensitive and gives due consideration to any special needs.
* Demonstrate an understanding of the Mental Health Act 1983 as amended 2007, in particular Part 3 of the Act and other legislations such as Safe Guarding Children & Adults, DOLS, Mental Capacity Act 2005 and others relating to care of service users.
* To undertake and assist in the organisation of the day to day smooth running of the service.
* Contributes towards the ongoing quality of service and care and identifies any risk issues in relation to the patients/clients health and social care.
* To be able to work and travel across a geographical area or across integrated teams.
* Inform the team / person in charge of the patient’s physical/ psychological condition.
* Recognise and respond appropriately to aggressive behaviour.


Management responsibilities

* To exercise good personal time management, punctuality and consistent reliable attendance.
* To assist in maintenance of a safe environment for patients, visitors and staff.
* Monitor and maintain stock levels.
* Contribute to cost efficiency saving within the team area.
* Contribute to the administrative duties required by the team area.


Clinical

* To provide interventions to service users within the community mental health teams under guidance of members of the MDT.
* To working autonomously within the parameters of the role, undertaking day to day clinical prioritisation and work planning.
* To be an integrated team member of the community mental health team, working with the MDT to address goals for service users, and facilitate effective engagement, intervention and support to enable the individual to be as independent as possible and return to the care of their GP.
* To work in collaboration with service users, families and others in the persons network, promoting the principles of recovery, choice and personalisation.
* To meet expectations and quality standards in relation to intervention, care planning and risk management.
* To inform care plans and assessments and onward referrals, offering advice to service users, carers and MDT colleagues with support.
* To assess risk and develop risk management plans in collaboration with service users, which are reviewed and adjusted in line with the service user’s needs alongside the MDT.
* To ensure an understanding of the Care Act and Safeguarding processes and that key processes are effectively followed in the team.
* To attend all relevant clinical meetings as a member of the MDT.


Research and Training

* To assist in the design and implementation of service evaluation and development projects as required.
* To attend meetings in which service developments are planned and discussed.
* To assist in the design and implementation of audit and research projects.
* To undertake data collection, analysis, the production of reports and summaries, using IT and statistical programmes.
* To adhere to all Trust policies and procedures.
* To be aware of the Trust Policy on the Code of Conduct for Non-Registered Health and Social Workers.
* To attend and participate in regular personal development, action plans and appraisal systems as the training programme indicates, with the practice trainer, personal tutor and line manager.
* To attend all mandatory training as required by Trust policies.
* To attend appropriate training courses and keep up to date with developments within the practice area.
* To attend training programmes both commissioned or provided by the Trust.


Communication

* Maintain confidentiality of Information in accordance with Trust Confidentiality Code of Practice Policy.
* Contribute to the effective communication process of service users, carers, family, friends and staff colleagues.
* Communicate appropriate information to and from other department/ service areas as required.
* To attend and participate in team meetings and contribute ideas to the Multi-professional team.
* To use and complete care plans and pathway documents appropriately.
