Team Leader – Community Care Services
Location: Staffordshire & Telford
Salary: £26,000 per annum + Paid Mileage (32p per mile)
Hours: Full-time, alternate weekends required
Reports to: Care Managers

UK Phoenix Healthcare is seeking a Community Team Leader to join our growing team, supporting our commitment to delivering exceptional care in the community.
Key Responsibilities
* Conduct regular spot checks and aftercare visits to ensure high-quality standards are maintained.
* Complete medication observations, audits, and counts in line with regulatory and company requirements.
* Provide supervision and mentorship to field care staff, supporting new team members during their induction period and assessing their readiness for lone working.
* Review documentation, charts, and care notes to ensure accuracy and compliance.
* Assist with on-call duties, covering care calls when required during sickness or absence.
* Complete comprehensive reports and documentation within set deadlines.
* Work closely with Care Managers to identify areas for improvement, contributing to service development and quality assurance.
About the Role
This position is predominantly field-based with one day per week in the office. You will play an integral role in ensuring care delivery across the community meets company and regulatory standards.
Requirements
* Previous experience in a senior/community care role is essential.
* Strong knowledge of community-based care and compliance requirements.
* Excellent communication, leadership, and organisational skills.
* Ability to manage workloads effectively while maintaining high standards of care.
* Full UK driving licence, reliable vehicle, and willingness to travel across Staffordshire and Telford.
* Must have the right to work in the UK (no sponsorship available).
