Responsibilities
* You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs of patient and set realistic goals.
* You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person's services.
* CPD is actively supported through regular supervision and staff training.
Location and Team
The team is based at Carew House, Bodmin but we cover the geographical area of North Cornwall, from the Devon border down the north coast to Wadebridge through Bodmin and Lostwithiel to Fowey, Par and St Blazey on the south coast.

We\'re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people\'s physical and mental health. We also provide specialist support to people with dementia or a learning disability. We are a people organisation and people matter to us. Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
Opportunity
An exciting opportunity has arisen for an enthusiastic and experienced AHP to join the Homefirst team in North. This position is open to Physiotherapists who have experience of working in the community and wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team.
The Homefirst team is part of the integrated community team in the North comprising of registered and non-registered nursing, therapy and administrative staff working collaboratively with partner organisations to support the intermediate care needs of our local population. This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly. We are committed to supporting our dedicated staff, so only current employees of the following organisations can apply:
