An exciting opportunity has arisen for an experienced Paediatric Nurse to join our well-established Paediatric Cystic Fibrosis (CF) Clinical Nurse Specialist (CNS) team at the Evelina London Children’s Hospital.
The successful candidate will work as part of the hospital and home care service as a highly specialised nurse working with a specific group of children and young people with Cystic fibrosis. The post holder will be expected to deliver care both in the community and in the hospital setting.
The post involves working autonomously with children and families on the ward, in clinics on the Royal Brompton Hospital site and in the community carrying out home and school visits. You would also support nursing colleagues in other trusts and in the community as well as supporting the child to access school.
We are looking for a proactive, self-motivated experienced paediatric registered nurse with a passion for their career to take on the role of Clinical Nurse Specialist within the Paediatric CF team. Ideally you will have some experience looking after children with CF or other chronic respiratory conditions. You will be sharing our Trust values, are enthusiastic, professional, caring, and compassionate with clinical competence caring for our children.

Main duties of the job
As a member of the Paediatric Cystic Fibrosis team you will utilise your developed specialist knowledge and skills from the ward environment to support specialist advice, counselling and supportive care to patients and their families.
You will support the link between the CF service and the ward areas; providing education to both children, families and staff, supporting seamless admission and complex discharges. You will also work to support the wider multi-disciplinary team to optimise condition management in the community; discharge planning, communicating with schools, nurseries, health visitors, local hospitals and safeguarding.
The role will develop your skills as an autonomous practitioner whilst developing your specialist knowledge and skills to be able to contribute to service improvements though audit, service reviews, projects and teaching.
The successful candidate will work in collaboration with an existing CNS team to act as a resource of expert advice for other healthcare professionals and must be competent to deliver health promotion and education to parents, their families and others.
To use specialist knowledge to effectively communicate with families, providing education, reassurance and on-going support to children and families about their diagnosis, treatment options and outcomes.
Educate and empower children and families to learn the skills to self-manage their condition and care.

Detailed job description and main responsibilities
-Independently manage a group of patients within the specialty whilst
working as part of the multidisciplinary team, delivering specialised
individualised and personalised direct patient care
-Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care.
-Responsible for the correct administration of prescribed medication.
-Be responsible for assessing and recognising emergencies in the
specialty. To interpret information and take appropriate action and to lead
others to do the same.
- Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
- Ensure that patients’ receive high quality clinical care and a good patient
experience, having regard for their customs, religious beliefs and
doctrines.
-Recognise and avoid situations that may be detrimental to the health and
wellbeing of the individuals.
- To provide leadership, support and guidance to staff in monitoring the
planning delivery and evaluation of individualised specialist care.
- Is wholly accountable for his / her practice in line with the NMC code of
professional conduct and takes every reasonable opportunity to sustain
and improve his / her knowledge and professional competence.
- Ensure safe and effective care for complex patients requiring advanced
specialist clinical skills and responsible for carrying out specialist nursing
procedures to area.

-Ensures the cultural and spiritual needs of patients are recognised and

-Participate in case conferences in relation to the management of patients, ensuring the coordination of ongoing care.
-Ensure staff take precautions to minimise the risk of cross infection in
relation to the safe disposal of body fluids (including blood).
-A key member of the multidisciplinary team in supporting patients and their carers during the delivery of bad news such as caring for the terminally ill patients and the event of death.
- Assess educational and information needs for patients and families, and
devise plans to ensure need are met.
-To be an effective communicator with patients/carers/families regarding
disease treatment and management.
-Promote high quality clinical care, and assist in the development of the
specialist service, through the use of evidence based practice and clinical
standards
- Provide a seamless, high quality service from referral through to
assessment, diagnosis, treatment and review.
