Bramley Village Health and Wellbeing Centre is looking to recruit a Practice Nurse to work between 30 to 37.5 hours per week. The surgery has 3 branches, this role will be based primarily at Middleton Park Surgery, but occasional travel across the group of surgeries may be needed. Our sites are Bramley Village Health and Wellbeing Centre, LS13 2BL, Middleton Park Surgery, LS10 4HT and Cottingley GP Surgery, LS11 0HJ.
The surgery has recently restructured its nursing team by building a fresh and enthusiastic team of Practice Nurses, HCAs and Nursing Associates that are committed to working alongside our team of GPs, ANPs and Medical students to provide outstanding patient care.
We are looking for applicants with accredited Chronic Disease Management (Respiratory and/or Diabetes) Qualifications to provide effective Long Term condition management across our 3 sites which serves 17,500 patients.
The organisation is also committed to providing and supporting staff development via internal and external training. The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, minor ailment treatment, immunisations, and health education. The Practice Nurse will work under the guidance and supervision of our team of advanced nurse practitioners and our organisational nursing director in order to contribute to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.
Responsibilities
 * Perform clinical procedures and manage patient follow-ups, ensuring care is delivered in accordance with the latest clinical guidelines and within the scope of practice.
 * Provide chronic disease management (Respiratory and/or Diabetes) across three sites.
 * Deliver minor ailment treatment, immunisations, and health education.
 * Support patient assessment, treatment planning and ongoing management in a primary care setting.
 * Work under the supervision of advanced nurse practitioners and the organisational nursing director.
Qualifications
 * Accredited Chronic Disease Management qualifications (Respiratory and/or Diabetes).
 * Experience in primary care and multidisciplinary team environments is desirable.
About the Organisation
The Fuller and Forbes Healthcare Group is a primary care provider with practices currently in Leeds, Devon, Burnley and Gateshead. The Leeds based practices provide care to 17,500 patients across Leeds at locations in Bramley, Middleton and Cottingley. The surgery is made up of a diversified clinical team consisting of GPs, GP Registrars, ANPs, ACPs, and a nursing team, with clinical supervision and mentorship by GPs to medical students.
The organisation is patient-driven and has used patient feedback in the past to develop innovative services such as a bespoke smart phone app, patient-centred practice websites, joint working with the third sector and a bespoke e-consultation service.
