Job Outline

Position: Community Team Supervisor (CTS)

Report to: Registered Locality Manager (RLM)

Direct reports: Carers /keyworkers

Key Tasks

· Promoting and growing the business within the locality area.

· Responsible for the Quality care provision for care recipients.

· To carry out Risk assessments and Initial care recipient visits.

· To ensure RA & Care plans are regularly updated and appropriate to the care recipients needs.

· To promote outcome based provision and ensure the care recipient is enabled where appropriate.

· To carry out their staff supervisions and record appropriately in office system.

· To develop and motivate their team of staff working closely with them and providing support where appropriate.

· Meet with RLM regularly and give feedback on client risk assessments, care recipient issues, staff supervisions and staff issues.

· On-call duties.

· Promote the image of Abicare Services ltd and adhere to all requirements set by Abicare Services Ltd standards.

· General care duties and act as a Role model and mentor for all care staff.

General Care Duties

You will be required to have a flexible approach to your care duties. You will be required to cover periods of sickness, annual leave and training (if qualified) and to assist with covering shifts which may sometimes be outside your availability. You will be required to complete calls for new packages taken on to enable a thorough and precise Service Delivery Plan to be completed, which is person centred and outcome based. You will then complete a handover with the team to ensure the all staff involved in the care delivery are aware of the care recipients requirements.

To act as a Role Model and Mentor to all Care Staff

· To deal professionally and promptly at all times with team members.

· To ensure that all policies and procedures are adhered to whilst completing care calls

· To ensure the dress code is followed

· To ensure documentation is filled in correctly

· To ensure the team use electronic monitoring systems in line with company policy

· To ensure gloves and aprons are worn.

· To assist with carers induction training.

· To build an effective team working closely with the care workers to ensure they have the skills set to deliver care.

To carry out Risk Assessments and Initial Service User Visits

Initial care recipient Assessments and Risk Assessments are to be carried out in line with Company Policy.

On-call

Your locality office will require assistance with on-call duties throughout each period. On call duties will be completed on a Rota basis as set by the locality team.

Rostering

CTS will ensure all care recipient calls are covered appropriately. The CTS will develop an effective cohesive team to deliver quality care within their allocated area.

Attend and contribute to meetings, supervisions and appraisals

You will be required to attend quarterly reviews in the form of supervision, an annual appraisal with your line manager.

Whenever you are on duty you will be expected to promote and maintain Abicare’s image. The CTS will work closely with the third sector and specialist groups within this area, to ensure that clients have access to the services required and feel included in their community.

Cover holiday and sickness as required

As members of the locality office team are on holiday or off sick the CTS will be required to assist with the smooth running of operations in the locality and may be required to spend time covering office duties.

About The Role

The Person:

You will have:

* Previous experience within the social care sector -preferably Live-in/Domiciliary Care

* Sound knowledge CQC/CIW Regulations and person-centred care.

* Possess excellent organisational and time management skills

* Excellent IT and communication skills

* Full driving licence with car
