* Carry out Carers Assessments and work with service users and carers with assessed eligible social care needs in order to promote recovery or prevent deterioration.
* Work under minimal supervision, with adherence to Trust procedures and policies, and employ value based and socially inclusive practice.

About ELFT and Our Values

ELFT has long been recognised as a centre of excellence for mental health care, innovation and improvement. Our mission is to make a positive difference to people\'s lives by improving quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive. We are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.

Quality Improvement is a key part of our work to improve our services and refine how we do things. We launched our Five Year Strategy in April 2018, identifying four main strategic outcomes to improve: population health outcomes, experience of care, staff experience and value. This complements the NHS Long Term Strategy and sets out our direction of travel.

We are a diverse, flexible-working friendly organisation and welcome applicants who can contribute to flexible working patterns such as job share or part-time arrangements. We encourage lived experience of health difficulties and diverse backgrounds to enrich our teams.
