Responsibilities:
* Provide emotional support and counseling to patients and their families.
* Develop and review treatment plans in collaboration with the healthcare team and participate in multidisciplinary team meetings to ensure patient care is coordinated.
* Coordinate patient discharge planning to ensure a smooth transition from hospital to home or other care facilities.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
