Following new national funding to expand OPAT services, we are offering an exciting opportunity for an experienced and motivated pharmacist to join our Acute Services team. As Advanced Clinical Pharmacist for OPAT, you will deliver expert pharmaceutical care to patients receiving outpatient antimicrobial therapy, supporting the expansion of novel pathways and improving patient clinical outcomes. You will play a key role in running pharmacist-led clinics, reducing wait times, and identifying patients suitable for outpatient management.
Key Responsibilities
* Lead and deliver pharmacist-led outpatient clinics for OPAT patients.
* Provide expert pharmaceutical care and clinical input for complex antimicrobial therapy.
* Support the expansion of new pathways, working closely with multidisciplinary teams.
* Contribute to governance, guideline and pathway development, and medicines optimisation.
* Identify and deliver opportunities for efficiencies and cost savings in medicines use.
* Support quality improvement and service development initiatives.
* Ensure compliance with best practice and national standards.
