CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are looking to recruit a Specialist Community Physiotherapist or Occupational Therapist to join our Urgent Community Response (UCR) team for South West Hertfordshire, covering the Rapid Response and Discharge to Assess pathways.

The role involves working with patients in their own home to ensure safe transfer from hospital to home, and prevent admission to hospital. We work in an interdisciplinary manner between occupational therapy and physiotherapy to enable a holistic approach when assessing patient's needs.

Successful applicants will be part of an enthusiastic, patient focused, holistic team treating patients in their own homes. Our Multi-disciplinary team comprises of Occupational Therapists, Physiotherapists, Nurses, Healthcare assistants, therapy assistants, Paramedics, Advanced Clinical Practitioners, GPs, as well as support from administrators.

We work collaboratively with GPs, acute hospitals, Social care, Hospices and other third sector organisations. You will have the opportunity to work with patients with a wide range of clinical conditions.We have a well-established structure for clinical supervision and opportunities for practice development including in-service training. This will provide you with fantastic opportunities to enhance your existing knowledge and skills.


Main duties of the job

To be an active member of the clinical team providing comprehensive, holistic assessments.

Work within professional standards and clinical guidelines, promote best practice and undertake all aspects of clinical duties as an autonomous practitioner including assessing, treating and managing own caseload of patients and maintaining professional documentation.

Build and maintain relationships with other local services to facilitate appropriate referrals and high quality healthcare.

To deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes .

Actively participate in Continuing Professional Development to support your own development and the development of the service. Including participating in any audit or research being completed by the service.

To deputise for the Band 7 therapy leads when required

Job responsibilities

Complete comprehensive autonomous and multi- professional assessments. Develop and implement treatment plans for patients referred to the service, using evidence based outcome measures.

Perform overview assessments and complex specialist assessments as appropriate. To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with trust guidelines for sharing of information and informed consent. Agencies include General Practitioners, Adult Care Services, Healthcare Professionals, voluntary agencies and residential homes.

You will be responsible for gaining valid, informed consent for assessment, treatment and sharing information in compliance with legal and professional guidelines. To be able to interpret and follow the guidelines in circumstances where patients do not have capacity to provide informed consent.

Person Specification


Qualifications

* Degree/Diploma in Physiotherapy or Occupational Therapy
* Current HCPC registration
* Evidence of up to date CPD portfolio
* Member of the CSP/RCOT


Experience

* Broad rotational experience at a junior level including experience in Care of the Elderly, Orthopaedics, Respiratory, Musculoskeletal and Neurorehabilitation.
* Experience working as an autonomous practitioner assessing and treating patients with physical conditions.
* Experience of current PT/OT assessment and treatment techniques/modalities
* Experience treating adult and/or elderly client groups
* Previous experience of working in the NHS.
* Experience in Falls Prevention and community work
* Experience in supervising staff and students.


Skills

* Knowledge and understanding of complex medical conditions including multi-pathologies and falls.
* High level skills in the assessment and treatment of adults with complex multi-pathologies.
* Ability to apply core physiotherapy/occupational therapy skills across a range of clinical conditions with a client centred goal setting approach.
* Ability to problem solve, prioritise and demonstrate clinical reasoning.
* Ability to teach junior staff/students/peers through and in-service training programme.
* Ability to communicate clearly and effectively, in both verbal and written format including report writing skills.
* Good IT skills, including ability to use Microsoft word.
* Knowledge and understanding of the role of Social Services and voluntary agencies.
* Knowledge of relevant national frameworks and guidance e.g. NSF, NICE.
* Ability to effectively use Systmone and Microsoft teams.
* Knowledge and experience in research and audit.
* Knowledge in provision of minor adaptation equipment


Personal/Physical Attributes

* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
