About The Team
We are seeking a compassionate and skilled Advanced Practitioner to support individuals with dementia within a mental health inpatient unit. This role is vital in ensuring that patients receive holistic, person-centred and timely assessments, in line with the Care Act 2014, to support discharge planning.
About The Role
You will work across the East and North Hertfordshire area covering the inpatients units where these individuals are located. Your office base will be in Farnham House, Stevenage; however you will be expected to attend the relevant inpatient unit to carry out assessments and attend CPA meetings.
Key Responsibilities:
* Undertake social care assessments for individuals with dementia admitted to inpatient mental health units.
* Work collaboratively with multidisciplinary teams to develop and implement care and discharge plans.
* Advocate for patients’ rights and preferences, ensuring their voices are heard in care planning.
* Liaise with families and carers to ensure continuity of care post-discharge.
* Maintain accurate and timely records in line with professional and legal standards.
* Contribute to safeguarding processes and risk assessments.
About You
Essential:
* Qualified Social Worker and registered with Social Work England.
* Actively undertakes a specialist role i.e. Practice Educator, Approved Mental Health Practitioner, Best Interest Assessor - or agreement to undertake a specialist role within the first year of the role.
* Experience working with older adults and individuals with dementia, preferably in mental health settings.
* Strong understanding of the Care Act, Mental Capacity Act, and safeguarding procedures.
* Excellent communication and interpersonal skills.
* Ability to work autonomously and as part of a multidisciplinary team.
