You possess a proven track record of collaborating with multidisciplinary professionals, participating in ward rounds, and handling critical legal and safeguarding processes. What the Job Involves In this role, you will be an integral part of the LB Sutton Mental Health team, focusing on hospital discharges from mental health hospitals. You will carry out Care Act assessments, reviews, and care planning, and work closely with housing and brokerage services. Additionally, you will be involved in Court of Protection applications, Mental Health Tribunals, and Safeguarding Enquiries, ensuring a high standard of care for complex mental health cases. The role demands travel within and outside the borough to meet the needs of clients.
